==== Absorption and bioavailability ==== The oral bioavailability of estradiol is very low. This is due to the fact that estradiol is poorly soluble in water, which limits its dissolution and absorption, and is additionally subject to extensive metabolism during the first pass through the intestines and liver. Estradiol is micronized and/or conjugated with an ester, as in estradiol valerate or estradiol acetate, to improve its oral bioavailability and potency. Micronization decreases the particle size of estradiol crystals and hence increases the surface area for absorption, thereby improving the rate and extent of absorption. In addition, there is an improvement in metabolic stability. Oral micronized estradiol consists of more than 80% of estradiol particles micronized to a size smaller than 20 μm in diameter, or to about 1 to 3 μm on average. All oral formulations of estradiol available today are micronized, and oral estradiol valerate tablets also seem to be micronized. Oral non-micronized estradiol and oral micronized estradiol do not appear to have ever been directly compared in a study. Both have been assessed independently however, and have been found to produce significant estrogenic effects. Micronization of other poorly water-soluble steroids such as spironolactone and norethisterone acetate has been found to increase their potency by several-fold.
== Clinical significance == Gastroesophageal reflux disease (GERD) is a common disorder that occurs when stomach acid repeatedly flows back into the esophagus, this backwash of acid (reflux) also known as heartburn can irritate the lining of the esophagus. Most people are able to manage the discomfort of GERD with lifestyle changes and medications, notably proton pump inhibitors, and H2 blockers. Antacids may also be used to neutralise gastric acid. Sometimes, surgery may be needed to ease symptoms. Chronic inflammation of the gastric mucosa can lead to atrophic gastritis resulting in a decreased secretion of gastric acid, and consequent digestive problems. In hypochlorhydria and achlorhydria, gastric acid is either low or absent, respectively. This can potentially lead to less protection against ingested pathogens such as Vibrio or Helicobacter bacteria. In Zollinger–Ellison syndrome gastrin levels are increased, leading to excess gastric acid production, which can cause gastric ulcers. Hypercalcemia also increases gastrin and gastric acid and can cause ulcers. In diseases featuring excess vomiting, hypochloremic metabolic alkalosis (decreased blood acidity by H+ and chlorine depletion) may develop.

A serous membrane lines the pericardial cavity of the heart, and reflects back to cover the heart, much like an under-inflated balloon would form two layers surrounding a fist. Called the pericardium, this serous membrane is a two-layered sac that surrounds the entire heart except where blood vessels emerge on the heart's superior side; The pleura is the serous membrane that surrounds the lungs in the pleural cavity; The peritoneum is the serous membrane that surrounds several organs in the abdominopelvic cavity. The tunica vaginalis is the serous membrane, which surrounds the male gonad, the testis. The two layers of serous membranes are named parietal and visceral. Between the two layers is a thin fluid filled space. The fluid is produced by the serous membranes and stays between the two layers to reduce friction between the walls of the cavities and the internal organs when they move with respect to one another, such as when the lungs inflate or the heart beats. The sequencing platform to be used is chosen depending on different factors such as laboratory's research objectives, personal experience and skill levels. So far, the Illumina MiSeq system has proven to be the most commonly used platform for infectious disease research, pathogen surveillance, and pathogen discovery in research and public health. The instrument is compact enough to fit on a laboratory bench, has a fast runtime as compared to other similar platforms, and has a strong user support community. However, with further improvements of this technology and with additional error reduction and software stabilization, the MinION may be an excellent addition to the arsenal of current sequencing technologies for routine surveillance, especially in smaller laboratories with limited resources.
